You agree that Your information given to Us is a true and accurate record. You agree to be responsible for all the payment of fees and understand that payment is due at the time of service. Any expenses, costs of disbursements incurred by Dental at Keys shall be paid by the responsible party listed in the details You have given. You further acknowledge that failure to attend any appointment without notice may result in a fee and a deposit requirement for any future appointments (see below for details). You have read and agree with the privacy statement below. Forms will be electronically signed and a copy will be kept digitally. Any paper forms that have been filled will be scanned and digitally kept. You agree to this process.

Changes to your appointment
While we endeavour to provide patients with a courtesy reminder for their appointments, it is ultimately the patient’s responsibility to remember the appointment time. Our practice requires 48 hours notice for changes for all appointments. Failure to give adequate notice may result in a fee charge.

General privacy Statement
Dental at Keys respects your right to privacy and considers all of the information you have provided in this form to be personal information for the purposes of the Privacy Act 1988 (C’th) as amended (“Privacy Act”).
We collect the information in order to provide you with dental services. We will keep your information secure and confidential. If necessary, we may pass your information on to other health practitioners for a second opinion or referral purposes. We may also be required by law to provide your information to outside agencies. Dental at Keys, its related companies or agents (“Related Persons”), may also use this information to promote health and related services to you or for other purposes permitted under the Privacy Act.
Where possible we only collect your personal information directly from you and where that is not reasonable practicable we may collect your personal information from other sources. Dental at Keys may collect personal information directly from you when:
• You complete a medical history form;
• You request information concerning Dental at Keys’ services in person, by phone or online.
In addition we may collect personal information from Related Persons to assist them in developing and promoting health related products and services that may be of interest to you (unless you ask us not to).
We may take images of your teeth before, during and after treatment. These images will be mainly used for record keeping and to assist us in treatment planning. You acknowledge that these images may be used in a practice portfolio to showcase examples of dental work to other patients and your identity will remain anonymous.
By providing your personal information to us in this form or by other means, you acknowledge and agree that Dental at Keys may:
• Collect and use your personal information to provide health and related services to you;
• Collect and use your personal information to contact you for market research and to provide you with information and offers about health-related products and services offered by Dental at Keys and Related Persons; and
• Disclose your personal information on a confidential basis to Related Persons who may contact you for promotional and informational purposes in relation to health-related products and services.
Our staff may contact you on available telephone numbers and email addresses. When our staff contact you and you are not available, they may leave messages which identify the caller or sender and the purpose for which the communication is made.
Whenever you are provided with market research or marketing information by Dental at Keys or Related Persons you will be offered the opportunity to inform us if you do not want your personal information to be used for those purposes in the future.
